ADMISSION FORMS

REQUIRED FORMS:

There are several forms you are required to complete prior to your child's admission.   All forms must be filed out in COMPLETION.   Incomplete registration forms will result in delay of preschool services.


Preschool Policies and Procedures Handbook/Contract
  • The first three pages are a questionnaire about your child.
  • All of our policies and procedures are included in the handbook/contract, so please make sure to read the entire form and initial where applicable. If you have any questions or there is something you do not understand, please ask prior to initialing or signing.
  • The last two pages need to be signed AND notarized. (Notary is typically free at the bank you use.)
Preschool Enrollment Register
  • ALL Lines must be filled out. Any that do not apply need to have N/A or NONE written in.
Preschool Financial Agreement
  • Please fill in as much as possible. If you have any questions or there is something you do not understand, please ask prior to initialing or signing.
Permission Authorization
  • Please fill in as much as possible. If you have any questions or there is something you do not understand, please ask prior to initialing or signing.
Authorization for Pick-Up
  • Please fill in as much as possible. If you have any questions or there is something you do not understand, please ask prior to initialing or signing.
Risk Waiver
  • ALL Lines must be filled out and signed.
Immunization Information
  • A Certificate of Immunization form must be current and updated yearly (more frequently for infants). All children must be current on their immunizations. If there is a signed exemption to immunizations, the child may need to be excluded from childcare if there is an outbreak of a vaccine preventable disease that the child has not been immunized for. *WAC 170-296-0850.

OPTIONAL FORMS:

Publication Release Form
  • Please fill out both the student and parent section.

The following forms are not required upon registration, but will be required once children are enrolled if any medication will be need to be administered:
*WAC 170-296-870 AND *WAC 170-296-880

A few things to keep in mind for the following forms:

  • A physicians signature is required once a year for all prescription AND non-prescription medications for children 24 months and under.
  • Parent's signatures are required MONTHLY for all children in order to administer any medication to children. If forms are not signed, we will not be able to administer any medication-you will have to either pick up the child, or you will be asked to come administer the medication yourself.
Tylenol / Other Pain Reliever Instructions
  • For administration of Tylenol or any other Pain Reliever.
Benadryl Instructions
  • For administration of Benadryl due to allergic reactions.
General Medication Form
  • For administration of any other medication besides Tylenol/Pain Reliever, or Benadryl.
Repeat Medication Form
  • For administration of any medication that requires being given repeatedly, such as asthma medication.
Allergy Information Form
  • Only use this form if your child has known, documented allergies.
  • We also need a doctor's note attached to this form.
